Declaration of payments made by an individual to a charity
For completion by the donor:

•Please reclaim Gift Aid taxes on my donations. This includes all donations for this year and the previous four years, plus any future donations unless I notify you otherwise.
•By digitally signing this form, I confirm that I am a UK taxpayer. I understand that I must pay an amount of Income Tax or Capital Gains Tax for each tax year that is at least equal to the amount of tax that all charities that I donate to will reclaim on my gifts for that tax year. I understand that other taxes, such as VAT and council tax do not qualify and that St Michael’s Church Centre Ltd will reclaim 25p on every £1. I will tell St Michael’s Church Centre Ltd if I am no longer a taxpayer.
